Sponsorship form templates?

Does anyone have a basic sponsor form template?

I know it's cheating and once I've had a coffee and got rid of DS's pooy nappy I should be able to concentrate:cheesy: but had to ask, mostly to save time!

Would you not just need a brief description about the event and them to make a table underneath with headings for name, address, amount - per whatever is being sponsered and a total amount colum for those who make a donation rather than sponser.

As part of our current fundraising drive to buy Mrs BigBadBosslady on owned, we are planning a sponsered bellydance on PooTube. The event will take place next saturday. Please sponser mummylisa80 in this event.

Name Address per hour total
Patti mumszone 20p [fill in after}
Mr T Lazytown - £5000


and then you would need a total collected line at the end.
